L’Chaim Eligibility & Enrollment

L’Chaim Adult Day Health Care (ADHC) provides medical, therapeutic, and social services through California’s Community-Based Adult Services (CBAS) program. We are a day care program designed to support older adults and adults with disabilities to live safely and maintain their independence.
To qualify, individuals must have certain medical needs, which are assessed through the CBAS program. Our team is here to guide you every step of the qualification process, from determining eligibility to completing enrollment.
Who is Eligible?
You may qualify for CBAS services at L’Chaim ADHC if:
- You are 18 or older and live with a chronic illness, disability, or functional limitation
- You can safely live at home outside of program hours (alone or with caregiver support)
- You need ongoing medical, nursing, or rehabilitative care during the day
- A physician recommends adult day health services based on your health needs
How Do I Enroll?
Join a Medi-Cal managed health care plan
If you’re not already enrolled, you’ll need to choose a plan (such as Anthem Blue Cross, San Francisco Health Plan, Health Plan of San Mateo, or Partnership HealthPlan of California)
Get a physician’s referral
Your doctor will complete a health evaluation and recommend services.
Work with us to complete your paperwork
Our team will help you gather medical records, complete forms, and submit everything to your health plan.
Wait for approval
Once your Medi-Cal plan approves enrollment, you can begin attending L’Chaim.
Private pay options are also available. Please inquire for more information on paying out of pocket.
